Accident / Illness / Death Cover

If someone passes away in my family, how do I submit a death claim?

1. It’s simple, CALL US on 0861 990 000 and we will guide you through the process.
2. If you meet all the Policy terms (for example you have met the waiting periods and the death
is not excluded for example where there is a pre-existing condition (see definition for pre-
existing condition under General FAQ section))
3. You will be required to provide us with a completed claim form, as well as the specific
documents / information listed below.
- A certified copy of the deceased’s ID;
- A certified copy of the death certificate; and
- A copy of the notification of death form completed by a doctor (otherwise
called a DHA-1663/DHA-1680 form);
- A letter of executorship/authority when the benefit is payable to an estate;
- If applicable, copies of the police; or motor vehicle accident reports, or
hospital records

4. In addition to the above specific documentation / information required; if a claim is
submitted:
i. by your Spouse, then we will require:
- A copy of your spouse’s ID;
- A copy of the marriage certificate
ii. when there is no Spouse; then we will also require:
- A copy of the ID of the person claiming;
- Letters of executorship; or
- An affidavit by the person claiming confirming they are the sole
dependent of the deceased or in circumstances where they are not the
sole dependent a supporting affidavit by another member of the
deceased’s family confirming that the claimant is so authorised
5. We reserve the right to request additional supporting documents from time to time should
we be unable to validate the claim with all the information requested above.
6. If we approve a claim; you will be required to provide us with a copy of your bank statement,
that clearly shows the name of the account holder, the account details as well as the Bank
date stamp.
7. All costs incurred in submitting a claim are for your account.

9. Failure by you to comply with our reasonable requests, non-cooperation in the investigation
of claims or the submission of specific claim validation documents / information may result
in rejection of your claim by the Insurer.

Does The Unlimited cover HIV/Aids?

Yes, we cover HIV/AIDS under our Death Benefit. Any claim that is approved where HIV/AIDS or a
related illness is the cause of death will need to meet the following:

- The HIV/AIDs must have been contracted after the start date of the
Policy
- You must have met a waiting period of 12 months. This is equal to a
period where you have paid 12 monthly debits in a row.

How do I submit a claim with The Unlimited if I've been admitted into hospital?

If you spend 24 hours or more in hospital (i.e. you are admitted to hospital) for an injury caused by an accident (the event), you can claim under your accident cash benefit. It is best to claim once you have been discharged and can claim for all the time you were in hospital for that event.

What is the claims process for an accident/death/illness claim?

Will I have to pay for this documentation?

Yes, you are liable for any cost incurred for the documentation needed for your claim.

Will The Unlimited pay the hospital directly?

NO. Once your claim is approved we will pay the proceeds of the claim directly into your Bank Account i.e. the account that we deduct your premiums. Only in the event of the Death of the Policy Holder will we pay an approved claim to another Bank account i.e. your Spouse, your Appointed Administrator or Executor.

How long does it take to assess and pay out a claim?

Once we have received ALL the required supporting documents together with your completed claim forms, your claim will be assessed and you will be informed of a decision regarding your claim within 72 hours. An approved claim is processed and paid within 24 hours.

What does it mean if I have an Accident Cash benefit on my policy?

When you, or a member of your family that you have included on the policy, are injured as a direct result of an accident and it is serious enough that you are admitted to hospital for a full day (24 hours consecutively) or longer, you have a daily amount of cover that you get for each day you are admitted up to the maximum cover limit of your policy.

What is a Helicopter Flight Bonus?

When you, or a member of your family that you have included on the policy, are injured as a direct result of an accident and it is serious enough that you are admitted to hospital for a full day (24 hours consecutively) or longer, you have a daily amount of cover that you get for each day you are admitted up to the maximum cover limit of your policy.
